Support for transition

We recognize that at transition time, learners benefit from high quality support in order to ensure that services  and young people are prepared and feel well informed. We work with professionals from the learners’ home area to plan services to meeting the young person’s needs. We have very positive relationships with Connexions services. Stockport Connexions service acts as a transitionlink with home services to ensure that all are included in the process.

Learners are supported to visit new placements during their course, they are encouraged to transfer skills learnt at the college to new settings and to working with new people.

Services are supported by staff at the college to offer positive support to people with complex needs. Training in specific aspects of support is offered according to individual needs. Therapy, teaching and residential staff share information and training in communication, audiology, mobility, care and personal preferences where required.

The Person Centred Planning approach used throughout the course enables learners to express and record preferences. Progress reviews include a personal statement from the learner that shapes their course and future planning.
